Fall 2010 Clinic registration info and FAQ

As stated in the another thread, the clinic dates for this fall are Oct 2, Oct 23, & Nov 6. Attached is the registration form and FAQ if you would like to either enroll a student or pass the information along.

Please note that for enrollments I need to be mailed the completed registration form along with a check.
